1. Two copies of valid passport

2. 4 recent passport photos

3. Original Birth certificate (long form for Canadians) (must be legalized and then Apostilled) An Apostille is a seal placed on an official document; the Apostille is widely accepted outside of the US and makes the document valid in all countries; and is issued by the Secretary of State's office in each state

• If you are a resident of the US, you need to contact the Dutch Embassy in your country of decent/origin to obtain the seal of Apostille. If you are born outside the US, your birth certificate must be legalized by the Dutch Embassy and the Ministry of External Affairs from the country of your birth.
• If you were born in the United States, your original birth certificate needs to be Apostilled by the Secretary of State office in the state in which you were born.
• If you were born in Canada; authentication is done by the Authentication and Service of Documents Section of the Department of Foreign Affairs. You must submit the original long-form birth certificate.
• Your birth certificate should be legalized and not be older than one year after they Apostille it

4. Certificate of good conduct (police record)

• A certificate of good conduct is obtained by contacting your local Police Department. Request them to conduct a criminal background check. Please send the printed report to us so that we may forward it with your immigration documents.
• This certificate should be completed no more than two months before going to Bonaire.

5. Proof of civil status (legalized with Apostille). This document cannot be older than 6 months.

• This is a statement of marital/civilian status. Follow the same procedures for the Apostille seal.
• The Civil Status needs to be legalized by the Dutch Embassy and Ministry of Foreign Affairs only if you are not residing in the USA.

6. Student Declaration

• You must complete and send only the Dutch version; the translated version is for your reference only.

7. Bank Declaration

• Please have your bank issue a bank statement to you. The bank statement must be on bank letterhead and must have your name on it. The bank account should show a minimum balance of $7,000.00 USD.

8. Model- I

• You must complete and send only the Dutch version; the translated version is for your reference only.

Married students can bring their family with them to stay as tourists for a maximum of three months. If the students wish their family members to stay longer, they must apply to the Census Office for a renewal of the permit.
